The covered entity (CE), Advantage Orthopedic & Sports Medicine, reported that it experienced a cybersecurity attack that affected the protected health information (PHI) of 1,914 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, claims and financial information, diagnoses and conditions, medications, and other treatment information.
