The covered entity (CE), Andrew Sauchelli, DMD, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 3,416 individuals. The PHI involved included names, dates of birth, Social Security numbers, physical addresses, diagnoses, claims information, lab results, and medications prescribed. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE retrained its workforce on email security and implemented additional technical safeguards.
