The covered entity (CE), Boundary Regional Community Health Center d/b/a Kaniksu Community Health, reported that it was notified by its business associate (BA), Aesto, LLC, that the BA experienced a breach involving access to its network by an unauthorized threat actor. According to the breach report, the breach affected approximately 88,000 individuals. The protected health information (PHI) involved clinical and demographic. The CE notified HHS, the media, and the affected individuals. OCR provided technical assistance to the CE regarding the HIPAA Rules and obtained documentation substantiating the CE’s notifications.
