Buffalo Surgery Center 01-25-603823 Breach Web Description The covered entity (CE), Buffalo Surgery Center, reported that it was the subject of an impermissible disclosure of protected health information (PHI) that affected the PHI of 64,000. The type of PHI affected included clinical, demographic and Other Treatment Information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
