Breach Notification Rule--Web Summary Community Health Center, Inc. 25-607334 The covered entity (CE), Community Health Center, Inc., reported a breach on their system that compromised the protected health information (PHI) of 1,060,936 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
