The covered entity (CE), Directions for Living, reported that it was the victim of a ransomware attack that compromised the protected health information (PHI) of 19,494 individuals. The PHI involved included names, dates of birth, Social Security numbers, and clinical and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and revised its HIPAA policies and procedures.
