The covered entity (CE), Entyre Care Massachusetts, Inc., reported that an employee inadvertently published files containing protected health information (PHI) to a publicly accessible repository. According to the breach report, the breach affected approximately 1,680 individuals. The PHI involved names, ages, Medicaid ID numbers, and other identifiers. The CE notified HHS, the media, and the affected individuals. In its mitigation efforts, the CE sanctioned the employee and implemented new technical safeguards to prevent a reoccurrence. OCR provided technical assistance to the CE regarding the HIPAA Rules and received evidence substantiating the CE’s notifications.
