The covered entity (CE), Equality Health, LLC, reported that an unauthorized individual gained access to the electronic protected health information (ePHI) of 9,240 individuals, through a phishing attack. The ePHI involved included clinical, demographic and other treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE reset passwords and conducted additional security awareness training. OCR provided technical assistance to the CE..
