The covered entity (CE), Humana, reported that an employee inadvertently mailed the protected health information (PHI) of 6,440 individuals to the wrong recipients. The PHI involved included names, addresses, medications, and diagnoses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
