The covered entity (CE), Pavilion of Bridgeview, reported that its business associate (BA) experienced a breach incident that compromised the protected health information (PHI) of 1,407 individuals. The PHI involved included names, addresses, birthdates, social security numbers, diagnoses, lab results, medications, and other treatment information.
