Personal Touch Home Care of Indiana, Inc., the covered entity (CE), reported that its business associate (BA) was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 3,593 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, lab results, medications prescribed, and other treatment information.
