Pope & Conner Consulting, Inc. OCR Transaction Number: 24-574297 Pope & Conner Consulting, Inc., a business associate (BA), reported that threat actors gained access to a BA workforce member’s email account via a phishing attack. The email account contained data that included the electronic protected health information (ePHI) of 6,251 individuals. The ePHI involved Clinical, demographic and other treatment information. The BA provided evidence that it notified covered entities, HHS and individuals on behalf of covered entities that accepted BA’s offer to provide notice. The BA took corrective actions in response to the breach including implementing multifactor authentication, contacting its third-party email provider to secure affected accounts, and resetting the passwords for all email accounts.
