The covered entity (CE), Region 4 South Mental Health Consortium, reported that its business associate (BA) experienced a ransomware attack affecting the protected health information (PHI) of 677 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims information, diagnoses/conditions, lab results, medications, and health insurance and other treatment information.
