The covered entity (CE), Seven Counties Services, Inc., reported that multiple employees were the subjects of an email phishing incident that affected the protected health information (PHI) of 132,609 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
