The covered entity (CE), The Stern Cardiovascular Foundation, reported that it experienced a ransomware incident that compromised the protected health information (PHI) of 270,645 individuals. The PHI involved included names, addresses, dates of birth, driver’s license or other identification numbers, Social Security numbers; diagnoses, medications, financial and claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website.
