The covered entity (CE), Washington University School of Medicine, reported that it experienced a hacking incident that affected the protected health information (PHI) of 1,056 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, and medication names. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
